Consent to Photograph

I give consent to A+ Pediatrics to photograph and use images of my child for the following purposes:

1. Marketing and Promotional Materials: These images may be used on A+ Pediatrics’ official website, social media platforms (such as Facebook, Instagram, Twitter, etc.), and other promotional materials for the purpose of promoting A+ Pediatrics’ services.

2. Educational Purposes: These images may be used for educational purposes within A+ Pediatrics, including but not limited to presentations, newsletters, and training materials.

I understand and agree to the following terms:

• The photographs may be used indefinitely by A+ Pediatrics.
• A+ Pediatrics may alter, modify, or edit the photographs as they see fit.
• I will not receive any financial compensation for the use of these photographs.
• A+ Pediatrics will not sell these photographs to any third party.

I understand that once the photographs are posted on the internet or used in other media, it may be downloaded by any computer user around the world, and therefore, I cannot control subsequent uses of the image.

I waive any right to inspect or approve the finished product wherein my child’s likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photographs.

This consent is valid indefinitely unless revoked by me in writing.
